Transition Care Program

Community Options Illawarra Inc. provides short term individual Case Management for people who are HACC eligible and have changed circumstances or have received a hospital discharge program and are waiting to access other/ongoing services.

Who can Refer?

Anyone can refer to Community Options Illawarra Inc. by contacting one of our offices directly. Following referral a Case Manager will contact you to determine eligibility, and discuss your needs.
