Community Options Illawarra Inc. provides individual Case Management to meet the specific needs of people with Dementia who have complex care needs and their carers to assist them to continue living at home.

COMMUNITY AGED CARE PACKAGES (CACP) FOR PEOPLE FROM CALD BACKGROUNDS
CACP's are funded by the Australian Government and provide flexible, planned and co-coordinated packages of community care services designed to help people who are frail aged with complex care needs to remain living at home.

COMPACKS PROGRAM
ComPacks is a 6 week case managed discharge program from acute hospitals, which aims to ensure that people returning home have appropriate care in place to meet their needs.

SUPPORT COORDINATION FOR OLDER PARENT CARERS
The program provides individual Case Management and the opportunity for older parent carers to tailor the support they need. The service will help parents maintain their independence, access appropriate services and adjust to any changes in their own needs or the needs of the person with the disability.
