Support Coordination for Older Parent Carers
The program provides individual Case Management and the opportunity for older parent carers to tailor the support they need. The service will help parents maintain their independence, access appropriate services and adjust to any changes in their own needs or the needs of the person with the disability.
Eligibility
Parents (grandparents) over the age of 60 who are caring for an adult child (grandchild) with a disability.
Aboriginal and Torres Strait Islander parents over the age of 45 who are caring for an adult child (grandchild) with a disability.
Who can Refer?
Anyone can refer to Community Options Illawarra Inc. by contacting one of our offices directly.
Following referral a Case Manager will contact you to determine eligibility, and discuss your needs.
Availability
The program is available to anyone who meets the eligibility criteria. Priority will be given to carers who are not currently accessing any support services.
