Community Aged Care Packages (CACP) for People from CALD Backgrounds

COMMUNITY AGED CARE PACKAGES (CACP)

CACP's are funded by the Australian Government and provide flexible, planned and co-ordinated packages of community care services designed to help people who are frail aged with complex care needs to remain living at home.

Eligibility

To be eligible to receive a CACP your needs must first be assessed by the Aged Care Assessment Team (ACAT). The local ACAT team can be contacted at Port Kembla Hospital on 4223 8282.

Who can Refer?

Anyone who has an approval by ACAT for a CACP.

Availability

Community Options Illawarra Inc. CACP program administers ten packages specifically for people from Culturally and Linguistically Diverse backgrounds who live in the Wollongong Local Government Area.
