Community Options Illawarra Inc. provides individual Case Management to meet the specific needs of frail older people and younger people with disabilities and their carers to assist them to continue living at home.
Eligibility
The program provides a flexible package of community care for people who:

are Home and Community Care eligible (HACC)
-
are of a cultural and linguistic diverse background (CALD)
-
have complex care needs
Who can Refer?
Anyone can refer to Community Options Illawarra Inc. by contacting one of our offices directly.
Following referral a Case Manager will contact you to determine eligibility, and discuss your needs .
Availability
The program provides individual case management and service coordination to meet the specific needs of people from CALD backgrounds and their carers.
Following referral to Community Options Illawarra Inc. a case manager will contact you to determine eligibility, assess your needs and develop an individual care plan with you.
